You need to devise your own CS Survey rather than copy one. It has got to be customized and tailored to your organizations and customers need. It has got to be un-biased and factful. The best survey is an average sized one with 5-6 questions and automated drop down ratings as that helps you in analyzing trends and emerging opportunities for your organization to improve on specific areas of service. You can include a comment section for personalized replies. I do have a variety of surveys with me, but I still feel you should have one of your own.
